When a team member requests leave through the form, you want to notify and have the right to approve/reject the team member's leave request through Teams as shown below.

First, you need to create a form on Microsoft Forms so that project members can request leave by filling out this form (of course).

Next, go to Power Automate and select as shown below

Next, just select the created form in the When a new response is submitted field

Once you have selected your form, click the + (Insert a new step) and look for "Get response details"

Next, find the Create an approval option and design as shown below.
