Web accessibility folks, especially those working in web apps... what is the most complex focus interaction on your site? What makes it complex, and how do you address it?

I'm especially interested in cases where you have to programmatically move the user's focus. Additionally, I'd love to know your stack, and whether you feel that stack helps or hinder your focus management.
